The deadline for 2016 CAPED Scholarship Application submissions is August 31, 2016 at 11:59 pm PST. There will be no deadline extensions, no exceptions.

2016 CAPED Scholarship Application

You CANNOT save your application and return to it at a later time. It is recommended to complete your standard essays, supplemental essays, and gather your necessary verification documents prior to beginning the online application process.

All attachments must be scanned copies uploaded in one of the following formats: .doc or .pdf. Please do not submit photographs of your forms/documents.

Incomplete applications will be automatically disqualified for consideration.

PART 3: VERIFICATION & RECOMMENDATION DOCUMENTS

LETTER OF RECOMMENDATION One letter of recommendation from a college/university instructor/professor (not a disability specialist or counselor) is required. This letter must describe your academic achievements and your ability to meet scholastic responsibilities. Save your letter of recommendation with the following naming format: Last Name, First Name_Letter of Recommendation. For example: Smith, John_Letter of Recommendation. DISABILITY OFFICE CERTIFICATION Download Disability Certification Form. Ask your college/university Disability Office to complete and sign form. Save your Disability Certification Form with the following naming format: Last Name, First Name_Disability Certification. For example: Smith, John_Disability Certification.
